Centers & Authorized Provider Locations

Our headquarters office has moved from the Portland, Oregon area to the south Fort Worth, Texas area.  We service both local and all long distance clients via this new office location.  All administrative related mail should be sent to our new Texas mailing address.

The Portland / Vancouver metro area continues to be served by 3 locations. There's also a service location in Lebanon, Oregon that serves from the Salem area down to Eugene/Springfield.
